The Role
We are seeking a motivated and detail-oriented Physical Therapy Assistant to join our dedicated team. The
Physical Therapy Assistant (PTA) works under the supervision and direction of a licensed Physical Therapist to
deliver evidence-based therapeutic interventions, support patient progress, and promote overall mobility and
functional independence. The ideal candidate is patient-focused, adaptable, and excited about providing
high-quality care in a rural setting with strong community relationships.
Key Responsibilities
● Provide physical therapy treatments as delegated by the supervising Physical Therapist, following
individualized care plans.
● Implement therapeutic exercises, manual therapy techniques, functional mobility training, and
modalities in accordance with state practice guidelines.
● Monitor, document, and report patient progress, changes, or concerns to the supervising PT.
● Educate patients and families on home exercise programs, injury prevention, and functional strategies
that support successful outcomes.
● Maintain accurate, timely clinical documentation within the electronic health record (EHR).
● Collaborate with multidisciplinary care teams—including primary care, behavioral health, and case
management—to ensure coordinated patient care.
● Assist in maintaining clinic equipment, treatment areas, and compliance with infection-control
standards.
● Support community health initiatives and participate in outreach relevant to rehabilitation and mobility.
● Uphold the mission and values of the FQHC, including commitment to health equity and serving
underserved populations.

Qualifications
Required:
● Graduate of an accredited Physical Therapist Assistant program.
● Current Oregon PTA license in good standing (or ability to obtain before hire).
● Current CPR/BLS certification.
● Strong communication and patient-education skills.
● Ability to work effectively with patients from diverse backgrounds, including underserved and
medically complex populations.
Preferred:
● Experience working in outpatient, rural, or community health settings.
● Familiarity with integrated care or multidisciplinary team environments.
● Excellent interpersonal and communication skills (verbal and written).
● Ability to work effectively in a fast-paced environment and handle multiple tasks simultaneously.
● Proficiency with computer systems and the ability to learn new pharmacy software quickly.
Compensation & Benefits
● Wage: $32 - $36, plus incentive compensation, DOE and certifications.
