Assertive Community Treatment Supervisor

What You Will Do

Wallowa Valley Center for Wellness is seeking a motivated, team-focused individual with supervisory experience to lead our close-knit and highly successful Assertive Community Treatment (ACT) team. You must enjoy working in an environment where each day brings new challenges and opportunities that you tackle together. In this vital role, you not only support an ACT team that regularly meets or exceeds its fidelity score but also bring a strong level of patience and compassion in relating with individuals with severe and persistent mental illness. You will provide comprehensive, whole-person care, tailored to each individual's unique needs and help them maintain their preferred level of independence. This could include but is not limited to skills training, counseling services, and case management. Additionally, you have the skills to track, record, analyze, report, and implement program improvements based on the fidelity requirements.

Your Responsibilities (include but are not limited)

Encounterable Services (20 hours/week)

  • Schedule and perform at least 8 individual therapy sessions each week and update 24 Mental Health Assessments and Treatment plans quarterly
  • Work closely with ACT clients in case management services (e.g. financial and housing assistance, transportation, mental health services, medical services, social services, personal care assistance, and educational services) as needed, approximately 4 hours per week
  • Facilitate and/or conduct training sessions (individually or in a group setting) every week to assist ACT clients in maintaining or achieving their desired degree of independence. Sessions will focus on daily living activities to help them develop and/or maintain self-management skills (e.g. personal hygiene, grooming, personal spending, maintenance of personal space and/or living environment, community access, and self-expression)
  • Assist ACT clients with medication management and monitor medical issues, following physician's orders for all medications and/or procedures, and providing documentation of these services
  • Enter billable services into an electronic health record weekly

Team Lead (16 hours/week)

  • Review ACT budget and develop policies and practices to increase efficiency and cut cost
  • Create a calendar system to track the timeliness of assessments, treatment plans, and other paperwork that is used consistently across the ACT team
  • Track weekly contacts of ACT clients and strategize with the team ways to improve and increase weekly contacts by facilitating daily team meetings
  • Research and develop collaboration opportunities to improve both the program and ACT clients' quality of life, including but not limited to vocational development, community integration, awareness, and access
  • Share program successes, challenges, and potential solutions weekly with your supervisor that ensure program improvement and growth
  • Provide monthly one-to-one supervision to ACT team members that support their professional growth
  • Other duties as assigned

The qualified ACT Supervisor we are seeking is an individual with the following qualifications:

Required

  • Qualified Mental Health Professional (QMHP) certification
  • Master's Degree in behavioral health
  • Two years of experience in crisis intervention and the direct treatment of individuals with mental illnesses
  • A valid driver's license
  • Ability to pass a criminal background check

Preferred

  • Licensure in the State of Oregon or be able to obtain
  • Previous supervisory experience

Knowledge, Skills, and Abilities

  • Ability to communicate effectively in both written and oral formats, including the ability to complete objective, accurate documentation in a timely manner
  • Ability to interact with individuals and community partners of various backgrounds, education levels, and socio-economic status
  • Must have strong computer skills including the ability to document in an electronic medical record, use electronic time tracking, and the ability to use Google Workspace specifically Drive, Meet, Sheets, word processing, and email
  • Ability to maintain confidentiality, comprehend laws, regulations, and policies, and the ability to follow such laws, regulations, and policies and report when they are being violated
  • Demonstrate the ability to lead highly successful teams in meeting metrics and requirements all the while supporting each individual's unique contribution

The Location
Hearts for Health Integrated Care Center offers medical, dental, and mental health services all under one roof in Enterprise, Oregon. This state-of-the-art facility houses two organizations, Wallowa Valley Center for Wellness and Winding Waters Clinic. The space has welcoming therapy rooms, spacious medical exam rooms, calming dental operatories, a teaching kitchen, and two community education spaces. Staff work in collaborative pods. There are also quiet spaces and wellness activities (e.g. basketball hoop, walking paths, bikes) to reset between appointments for staff. Additionally, the ACT Supervisor will work out in the community meeting clients in their personal spaces as well as community spaces throughout Wallowa County.
