Job Description
The Mental Health Clinician is a Licensed Clinical Social Worker (LCSW) or a Licensed Professional Counselor (LPC) or a Licensed Marriage and Family Therapist (LMFT) and is responsible for managing a panel of patients experiencing both acute and chronic mental illnesses, working closely with behavioral health professionals and a primary care team to provide patients with an integrated and comprehensive health care plan. Provides clinical recovery-focused services to patients with mental illness and substance abuse issues, and develops individual treatment plans in conjunction with the patient and other individuals, as indicated. Completes treatment assessments, session notes, plan updates, and other relevant clinical documents. May round on in-patients within hospital at provider’s request. May provide support for emergent situations when requested.
Requirements
Master's Degree in counseling, social work, psychology or a related behavioral health field from an accredited college or university.
LCSW, LPC or LMFT certification required.
Five (5) years of experience working directly with clients experiencing mental illness, in private practice model preferred.
This position has the ability to work onsite or remote in the State of Oregon.
