POSITION TITLE: Medication Aide and Cook
REPORTS TO: Residential Treatment Facility Administrator
FLSA STATUS: 40 hours a week- Full Time/Non Exempt
Position Summary- Med Aid .5 FTE/20 hours a week
Provides services and support to the residents in the RTF. MHTs assist residents with independent living
skills to maintain or increase their level of functioning. In addition to MHT functions, medication aides
administer medications and treatments under the supervision of nursing staff and per provider orders.
Essential Duties and Responsibilities
Meets and maintains the standards and procedures for the provision of care and services to
residents with severe and persistent mental illness in a licensed Residential Treatment Facility.
Read client support documents as needed upon reporting to work and make all appropriate entries
during and before the conclusion of the shift.
Prepares concise, complete, and timely documentation
Attends shift report
Cues and assists in maintaining appropriate and safe behavior as outlined in individual care plans.
Assists residents of either gender in personal self-management activities including: eating, toileting,
bathing, personal hygiene and grooming, cleaning, community access, personal spending,
socialization, recreation, skill acquisition, and self-expression.
Using the training and guidelines provided, transfer residents to and from wheelchairs, beds, chairs,
toilets, or where assistance with mobility is required.
Assists in serving and preparation of meals in accordance with MD orders, menus and dining plans.
Maintains clean, sanitary and safe conditions throughout the facility. This includes and is not limited
to: sweeping, mopping, vacuuming, scrubbing, window washing, bathroom cleaning, etc.
Helps residents meet goals set in Person Centered Service Plans created with Comagine
Attends staff meetings and training sessions as scheduled and reads and signs all required readings
in a timely fashion.
Communicates effectively both orally and in writing with other staff, administrator, therapists, and
health care providers by providing timely and accurate reporting of client and/or staff issues.
Utilizes TeamSTEPPS for communication
Maintains positive relationships with clients, their families, and the community.
Maintains confidentiality-adheres to rules and laws pertaining to client confidentiality and HIPAA
regulations.
Models the recovery model aspects, focusing on resident centered care, compassion and hope.
Models the WVCW mission and values
Is knowledgeable about WVCW policies and OARs that pertain to RTFs.
Ensure that cost effective practices are maintained in all areas.
Conducts and participates in evacuation/fire drills as assigned.
Reports any suspected violations of resident rights or abuse by another client or a staff member
according to policies and OARs regarding mandatory reporting. Such reporting is required for, but is
not limited to, suspected incidents of physical assault, neglect of care, sexual or financial
exploitation, which may include borrowing or removing property from the home.
Staying awake and alert throughout the entire shift.

Additional Duties and Responsibilities Required for Medication Aides
Administers medications and treatments in a timely and accurate manner per provider orders.
Documents all medications and treatments in the electronic record per policy and procedure.
Performs all tasks related to medication handling, administration, and storage per Oregon
Administrative Rules and program policies and procedures. Beyond medication and treatment
administration, tasks may include checking in medications, reviewing orders, reordering
medications, disposing of medications, managing medication errors
Performs delegated nursing procedures when applicable and documents per policy and procedure.
Qualifications
To perform this job successfully, an individual must be able to perform each duty satisfactorily as outlined in

Possess a satisfactory equivalent combination of education, experience, and/or training which
demonstrates the ability to perform the above-described job duties.
Basic computer literacy and ability to utilize multiple software platforms for documentation and
communication is required.
Ability to communicate effectively both verbally and in writing; comprehend laws, administrative rules
and regulations, and agency policies.
Valid Oregon driver's license
Pass a criminal background check
Pass a pre-employment drug screening
Current CPR and Food Handler's certification (to be obtained as soon as possible on hiring)
Other Experience, Skills, and Abilities
Experience working in long term care, assisted living, personal care, and/or mental health is
desirable
Possess patience, empathy, and flexibility.
Some knowledge of counseling techniques, theory, and practices for supporting mental health
clients is ideal.
Possess the ability to interact and relate well with residents, staff, and other professionals with
confidentiality, respect and dignity.
Ability to appropriately assist clients experiencing crisis situations.
Must be flexible in hours available to work
Physical Demands
This position requires an individual to be regularly available for work as scheduled. The physical demands
described here are representative of those that must be met to successfully perform the essential functions
of this job.
Professional and personal skills to cope with stress associated with work involving a high degree of
mental, emotional and physical demands.
While performing delegated tasks of nursing, this position will require fine motor skills and sufficient
vision to maintain accuracy.
Ability to bend, stoop, stand, kneel or crouch, push and pull, climb or balance, and walk up & down
the halls and/or stairs several times per shift.
Ability to lift and transfer clients using a two-person lifting technique.
Ability to lift up to 50 pounds.
Use office automation including computer and phone systems that require fine manipulation,
grasping, typing and reaching.
This position may involve working overtime, weekends, and evening and/or overnight (awake) shifts.
Infection control and universal precautions must be maintained to ensure disease will not be spread
or contracted.
Ability to self-regulate volume of voice to ensure confidentiality and to decrease unwanted stimuli in
the RTF.
Position Summary- Cook .5 FTE/20 hours a week
Your Responsibilities include but are not limited to the following:
- Prepare and serve meals and snacks in accordance with the published menu and recipe specifications and house schedule
- Implement special diets as prescribed by the primary care physicians
- Store, prepare, and serve food in accordance with Health Services Food Sanitation Rules
- Ensure that all dishes are washed and sanitized, and the kitchen and eating areas are cleaned after each meal
- Maintain proper cleanliness and organization of kitchen, walk-in cooler, refrigerator, freezers, and pantry
- Engage patients in social conversation and provide supportive listening
- Establish and maintain a positive working team environment with all registered nurses, providers, and residential facility staff and treat everyone with dignity and respect
- Participate fully in team meetings, training, and one-to-one supervision
- Provide feedback to oncoming staff regarding resident's interactions, activities, and behaviors at the end of your shift
- Report suspected changes in a resident condition and/or potential crisis situations to the charge nurse or floor lead
- Remain alert and provide direct line-of-sight monitoring of residents as needed
- Other duties as assigned

Work is performed in both an office environment and a residential environment. The noise level is usually
moderate, but occasionally may be exposed to loud noise such as raised voice levels and alarms.
This position may be exposed to the everyday risks or discomforts which require normal safety precautions
typical of such places as an office (i.e., moving mechanical parts, airborne particles, and electrical shock).
