Job Summary:
Clinic Care Manager provides direct and indirect patient care in the clinic and off campus. This individual provides premier care that reflects initiatives, flexibility, and responsibility indicative of professional expectation with a minimum of supervision in collaboration with providers. In conjunction with the primary healthcare provider they determine priorities of care based on physical, mental, spiritual, emotional, and psychosocial needs, as well as factors influencing patient flow through the system and care planning. The individual communicates with physicians about changes in patient’s status, symptomatology, and results of diagnostic studies. They are able to respond quickly and accurately to changes in condition or response to treatment.
Requirements:
Graduate from accredited school of nursing. Current Oregon RN License or ability to obtain. Current BLS certification. Performs all duties per the Oregon Nurse Practice Act. Ability to communicate effectively and work closely with all physicians, patients, clinic personnel, visitors, and outside agencies. Excellent verbal and written communication skills. Strong computer skills in EHR, word, Excel, and Outlook.
